The Crisis Addiction Response Team program CART dispatches a mobile outreach team to emergency departments at area hospitals to support individuals during a mental health or opioid related crisis. To avoid releasing the member back to the streets where the chance of recidivism within a short period is highly likely, Team picks the member up, brings them to a clinic where the outpatient team assists them as needed.

Team Wellness Center works with the hospital to ensure that the individual is getting the continuous care in an outpatient setting to reduce the likelihood of a going back to the ER.
